I. You are applying to 800 Degrees Phillips Place, LLC (the “Company”). The Company provides the staffing and management operations for a variety of hospitality locations. If you are successful in your application process and are offered employment, your employer will be the Company or affiliate; in any event, the Company remains the governing Company for policies and management.
II. The Company does not guarantee the number of shifts or hours you will be scheduled. The Company may reduce, increase or suspend your schedule as deemed necessary. The Company will provide employees with location assignments based on operational, personnel and internal needs. The Company does rotate employees to different locations on a periodic basis as it deems appropriate. Reassignment may occur at any time in the Company’s sole discretion. The Company may cease providing staff to any location at any
time and affected eligible employees will be placed on a list for any possible future placements.
III. The Company is committed to compliance with all applicable laws providing equal employment opportunities and makes employment decisions on the basis of merit. IV. All Company employees who receive gratuities are subject to the Company’s tip pooling policy as administrated by the site management. Tip pooling may be used as standard procedure for every shift or implemented at the discretion and necessity of the site management. The Company reserves the right to modify gratuity distribution formulas, tip pooling policies or alter the categories or positions of employee who receive benefit of tips individually received or pooled in its sole discretion. The site management will monitor, report, track, count and otherwise oversee the accounting of gratuities received and the distribution of those monies.
V. The Company evaluates performance and the ongoing employment of employees based primarily on, but not limited to:
1) An employee's professional and proficient execution of his/her specific assigned primary duties and secondary responsibilities.
2) An employee's ability to consistently demonstrate professional interaction with other employees, management and guests.
3) An employee's consistent professional, neat and clean appearance.
Your application for employment with the Company indicates your understanding of the above stated policies. You authorize the Company to verify and research all information you provided for consideration of employment. This includes any statements you make verbally during screening, interviews or in written responses. I hereby authorize 800 Degrees, LLC to thoroughly investigate my references, work record, education and other matters related to my suitability for employment and, further, authorize the references I have listed to disclose to the company any and all letters, reports and other information related to my work records, without giving me prior notice of such disclosure. In addition, I hereby release the Company, my former employers and all other persons, corporations, partnerships and associations from any and all claims, demands or liabilities arising out of or in any way related to such investigation or disclosure.
